Mood and Motion Therapy is required by law to maintain the privacy of your protected health information (PHI), provide you with notice of our legal duties and privacy practices, and follow the terms of this notice currently in effect.

Protected health information (PHI) is information that identifies you and relates to your past, present, or future physical or mental health condition, the healthcare services you receive, or payment for those services.

For Treatment

We may use and disclose your health information to provide, coordinate, and manage your care and related services.

Because Mood and Motion Therapy includes licensed therapists and associate therapists practicing under clinical supervision, your information may be discussed among members of your treatment team when necessary to provide quality care. This may include consultation between an associate therapist and one or more clinical supervisors, as well as other authorized clinical professionals involved in your treatment.

We may also share information with other healthcare providers involved in your care when appropriate and permitted by law to help ensure continuity and quality of care.

For Payment

We may use and disclose your health information to obtain payment for services provided. This may include sharing information with health insurance plans, billing services, or other entities responsible for payment of your healthcare services.

For Healthcare Operations

We may use and disclose your health information for healthcare operations necessary to run our practice and maintain high standards of care. Examples include:

  • Clinical supervision

  • Case consultation

  • Quality improvement activities

  • Staff training and education

  • Credentialing and licensing activities

Information shared for these purposes remains confidential and is used only as permitted by law.

Special Situations Related to Mental Health Treatment

As a psychotherapy practice, there are certain circumstances in which we may be required or permitted by law to disclose information without your authorization. These situations may include:

  • Suspected elder abuse or dependent adult abuse

  • When there is a serious threat of harm to yourself

  • When there is a serious threat of harm to another person

Our website and general email communications are not intended to be secure methods of transmitting protected health information.

Please do not submit medical records, diagnoses, treatment information, insurance details, or other sensitive health information through website forms, email, or online chat unless specifically instructed to do so through a secure platform.

For matters involving your treatment, records, or confidential communications, please contact our office directly.

Clients who utilize secure third-party platforms such as Headway, SimplePractice, insurance portals, or secure client portals may communicate through those systems in accordance with their privacy and security safeguards.

We are required by law to:

  • Maintain the privacy and security of your protected health information

  • Provide you with this Notice of Privacy Practices

  • Notify you following a breach of unsecured protected health information when required by law

  • Follow the privacy practices described in this notice

Changes to This Notice

Mood and Motion Therapy reserves the right to revise this notice at any time.

Any revised notice will be posted on our website and will apply to all protected health information maintained by our practice, including information created or received before the revision.

The effective date of the current notice will appear at the top of this page.
